The MedBridge program at HCR ManorCare consists of specialized medical and rehabilitation units within post-acute facilities that are designed for patients who are transitioning from hospital to home following a an injury, illness, or other medical event. These distinct, short-stay units cater to patients that require a higher level of medical and rehabilitation care in order to meet their goal of returning to independent or semi-independent living.

- Medical or rehabilitation diagnosis with prescribed treatment plan upon admission
- Expected discharge criteria of home or community

- Attending physician/nurse practitioner services available at least three days per week
- Specialty consulting physicians available as such as physiatry, pulmonology, cardiology, etc.

- Director of Care Delivery experienced in short stay, post-acute care, oversees unit and care delivery
- Dedicated nursing personnel for management of the admissions process
- RN Staffing on the MedBridge Unit 24/7
- Knowledge of physical assessment, post-acute and chronic medical and rehabilitation conditions
