Who is on Your Care Team? The Beacon of Hope Hospice Care Team is made up of trained professionals who put individual patient and family needs first. • Hospice physicians are collaborating with your primary physician to create the best plan of care. • Nurses are experienced in pain control and symptom management, to give you the support you need. • CNA’s help with personal care, such as bathing and personal hygiene, as well as light household duties if assistance is needed. • Social Workers and counselors are there to assist in meeting the emotional and supportive needs. • Chaplains offer spiritual support. • Bereavement specialists provide grief support for the family and loved ones. • Volunteers are there for many reasons to help the patients and family at this difficult time.
