At Manor Care Utica Ridge, our primary focus is caring for patients with complex recovery and rehabilitation needs in a homelike setting. Located on Commerce Boulevard in Davenport, Iowa, Manor Care Utica Ridge provides individualized post-hospital skilled nursing care in a comfortable environment. Our clinical and therapy teams are experienced in providing specialized care focused on your needs, interests, and ability. This commitment results in a smoother and safer transition throughout your recuperation.

The MedBridge program at HCR ManorCare consists of specialized medical and rehabilitation units within post-acute facilities that are designed for patients who are transitioning from hospital to home following a an injury, illness, or other medical event. These distinct, short-stay units cater to patients that require a higher level of medical and rehabilitation care in order to meet their goal of returning to independent or semi-independent living.

  • Distinct unit within the facility
  • 60 beds in size at Utica Ridge
  • Private or semi-private rooms
  • Clinical observation rooms available as necessary

    Amenities
  • TVs, phones, and WiFi available in every room
  • Distinct patient lounges and dining areas for MedBridge patients
  • Computer access available for patients and families
  • Selective menus available, commensurate with patient population
  • "Manager on Duty" program seven days per week, including holidays

    Admission criteria
  • Medical or rehabilitation diagnosis with prescribed treatment plan upon admission
  • Expected discharge criteria of home or community

    Medical services
  • Attending physician/nurse practitioner services available at least three days per week
  • Specialty consulting physicians available as such as physiatry, pulmonology, cardiology, etc.

    Nursing services
  • Director of Care Delivery experienced in short stay, post-acute care, oversees unit and care delivery
  • Dedicated nursing personnel for management of the admissions process
  • RN Staffing on the MedBridge Unit 24/7
  • Knowledge of physical assessment, post-acute and chronic medical and rehabilitation conditions
  • SBAR and INTERACT other protocols in place to minimize acute re-admissions
  • Discharge-oriented treatment protocols and patient education

    Rehabilitation services
  • Specialized rehabilitation equipment available commensurate with the needs of the short-stay population
  • Advanced modalities equipment and treatment protocols in place
  • Therapy training for complex medical and rehabilitation populations
