Many people associate home health with hospice, or assume that we are only here for urgent needs such as therapy or wound care after returning home from the hospital.

The truth is that Medicare and many other private insurances cover our services for preventative therapy—fall risk, for example. We also offer teaching on new diagnoses or medication management. Example diagnoses could include after-surgery care, congestive heart failure, cancer, pneumonia, stroke, back strain, diabetes mellitus, hypertension, hypotension, or heart failure.

Our team of caring physical therapists, occupational therapists, skilled nurses, and medical social workers can help you get the care you need without having to leave your home. We are experienced in providing a full suite of home health services and programs designed to achieve the best possible outcome for you or your loved one. If you require skilled medical attention to achieve recovery or optimum health, consider the home health option—and rest assured that you are in good hands.

We work with you and your physician to implement a care strategy to improve your health outcome in the comfort of your own home. Our clinicians will come directly to your house to provide the care you need. We also offer other services that include: In-home telemonitoring for chronic diseases, wellness check-in calls, and assisting with telephonic visits for your physician. We can be your first call for help.

Here in Central and Western Illinois, you often hear about the large hospital agencies for this type of care. Each person and each patient has a choice as to who provides them with home health care. At Northwestern Illinois Home Health, our quality care is backed by our CMS ratings with a five-star rating in quality of patient care and a four-star rating in patient satisfaction as of January 2020. You’re not a number to us, your part of our family.

To be eligible for care, you must be considered “homebound.” You may meet this standard if it is a “taxing effort to leave your home.” In most cases, you can still go to religious activities, senior centers, and even the beauty shop and still be considered homebound.

Medicare and/or Medicaid will cover the cost of home healthcare services for patients over 65 who meet either program’s eligibility requirements. Private insurance companies, managed-care organizations, and workers’ compensation plans may also pay for home health services.

If a patient with a history of a respiratory condition gets the flu, a doctor may call on us for extra help until they recover. Our unique continence control therapy program teaches non-invasive exercises in the privacy of your home that can significantly improve frequency and urgency.

This program can greatly improve quality of life, provide patients with the confidence to leave home more often, save money on essentials related to incontinence, and also help prevent falls that can happen when using the restroom at night.

Our services include an emergency response necklace, and you may qualify for aide services such as bath assistance; occupational therapy services such as home modifications or low vision tools; and social worker services such as financial assistance, counseling for long range planning, and help enrolling in other available programs.

We are here to serve our community and help you stay safe at home. Each plan of care is customized to your needs and scheduled under the direct supervision of your doctor.

We want to share a story from one of our most memorable LHC Group patients—Mr. Rice. Colonel Rice had to have emergency abdominal surgery and experienced complications that led to a severe decline in his health overall. Mr. Rice said “The image in the mirror was not recognizable.” Mr. Rice, a retired marine, had lost hope due to his acute health problem, surgery, and his current status. After home health services, Mr. Rice can now maintain and work on his farm and enjoy his retirement.
